Office Policies
Prescription refills: In the event you need a medication refill prior to your next appointment, please contact your pharmacy or request the refill from your Patient Portal. Please allow us 72 hours to process your refill request.
Appointment cancellations: We request 24-hours notice if you need to cancel your appointment.
Insurance: We file claims with most major insurance plans.
Credit Cards Accepted: We accept Visa, MasterCard, and Discover.
Discounts: We offer a discount for individuals with no insurance coverage.
Payment: Payment in full is due at time of service unless arrangements are made in advance with our billing department.
New Patients: We request that you arrive 30 minutes early to complete a thorough family and personal medical history interview; as well as to complete the new patient registration forms. Soon, you will be able to print the forms from the website and bring them completed to your visit.
Please bring your prescription bottles, over-the-counter medications and herbal or dietary supplements to every office visit.
