First Visit

In preparation for your first visit with our office, there are a few things you can do to get the most out of the time with your new provider.  Be sure to bring all medications (in their bottles), vitamins, and supplements to your visit. If possible, request that your previous physician forward your medical records to us.  But don't worry, if you can't get your records sent beforehand, we can always handle this request at your first visit.  With your help, we will be much better prepared to fully address your concerns. Many patients find it helpful to write down the questions they would like to ask.

When you schedule your first visit, you will also schedule an appointment for a phone interview to occur before your visit.  During this interview, our staff will get your medical, surgical, family, and social history input into our electronic medical record.  So please be ready with your information and medications/supplements for the phone interview.  By doing this before the visit, it greatly increases the time and communication with your provider during your visit. 

 

For your convenience, you can fill out all your new patient paperwork before your visit.  If you can’t fill it out ahead of time, you can do it at your appointment, but you will need to arrive 30 minutes prior to your scheduled appointment time in order to get the paperwork filled out before your time to see your provider arrives.

Either way, please don’t forget your insurance card and a photo ID.

Depending on your condition or concerns, your initial visit with us could take anywhere from 60 to 90 minutes.  After examining the records and information that have been supplied to us, your provider will conduct a thorough examination.  We encourage you to ask questions, and we will be glad to address any concerns you may have.
